Job Opportunity: Physical Therapist (PT) - Home Health

Location: Lakeland, Florida
Employment Type: Travel/Contract
Contract Length: 20 weeks

Job Description

• Provide skilled home health physical therapy services including assessment, cardiopulmonary/circulatory care, documentation, and use of equipment/devices such as gait belts, rolling walkers, sit-to-stand lifts, sliding boards, collars, supplemental oxygen, prosthetics, and wheelchair assessments.
• Manage ortho and neuro patients with home exercise programs, medication education, patient and family education, and safety/injury prevention.
• Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory patients, neuro, trachs, trauma, amputees (AKA, BKA), and ADLs.
• Perform 6-8 skilled home visits per day (minimum 6 points per day, with possible increase).
• Schedule 1-2 weekends per contract.
• Visits and hours are self-scheduled based on HomeCare office needs.
• Use laptop, MiFi for internet, and portable printers for documentation in BayCare systems.
• Work area includes Davenport/Haines City and Lake Wales/Winterhaven.

Required Qualifications

• Discipline: Physical Therapist (PT)
• Specialty: Therapy/Rehabilitation
• Experience: Minimum 1 year of home health experience required.
• Licenses/Certifications:

  • Valid Florida PT license required.
  • BLS certification required.
• Other Requirements:

  • Must have a valid driver’s license and reliable car for home visits.
  • COVID-19 vaccination required or approved exemption on file.
  • Flu shot required or declination on file; mask wearing mandatory at all times while at work.
  • Scrubs: Any solid color scrub.

Compliance and Documentation Requirements

• Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG, and education verification.
• Foreign equivalency evaluation if education was completed outside the US or Puerto Rico.
• Florida AHCA Level II fingerprint clearance required prior to start; must be current and on file.
• Physical exam within 6 months of start date.
• Negative TB test or chest X-ray within 10 weeks of start; TB questionnaire annually.
• Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• Standard 9-panel drug test within 30 days of start.
• Seasonal flu vaccination or declination with mask requirement.
• COVID-19 vaccination or approved exemption.
• Respirator medical clearance annually if respirator use is required.
• Two professional references from within the past 2 years.
• Employment application and skills checklist required.
• Photo for badge (clear, professional, solid background).
• Personal auto insurance current and active during assignment.
• Completion of BayCare Online Learning Center modules prior to start (4-8 hours).
• Compliance with BayCare Health MSP policies including dress code, tobacco policy, AHCA privacy policy, affidavit, and demographic information sheet.
• Medication and competency exams as applicable.
• Color blind testing required prior to start.
