Job Opportunity: Physical Therapist (PT) - Home Health

Location: Dunedin, Florida
Employment Type: Travel/Contract
Contract Length: 20 weeks

Job Description

• Provide home health physical therapy services with a focus on cardiopulmonary, geriatrics, neuro, ortho, and oncology patient populations.
• Conduct home skilled visits, typically 6-8 visits per day (minimum 6 points per day, may increase).
• Responsibilities include assessment, documentation, use of equipment and devices (crutches, gait belt, hospital bed, walkers, supplemental oxygen, etc.), home exercise programs, patient and family education, and safety/injury prevention.
• Must be able to self-schedule visits and hours based on office needs.
• Every 3rd weekend work required.
• Use of laptop, MiFi for internet access, and portable printers for documentation.

Required Qualifications

• Discipline: Physical Therapist (PT)
• Specialty: Therapy/Rehabilitation
• Experience: Minimum 2 years of home health experience required.
• Certifications:

  • BLS (Basic Life Support) required
  • State Physical Therapy license valid in Florida
• Other Requirements:

  • Valid driver’s license and reliable car for home visits
  • COVID-19 vaccination or approved exemption
  • Flu shot or declination with mandatory mask wearing at all times while working
• Prior travel experience is not explicitly required.

Compliance & Documentation Requirements

• Background checks including nationwide and state-level screenings
• Education verification and foreign equivalency evaluation if applicable
• Florida AHCA Level II fingerprint clearance required prior to start
• Physical exam within 6 months of start
• Negative TB test or chest X-ray within 10 weeks of start (or as required)
• Annual TB questionnaire completion
• Immunizations/titers required: Measles, Mumps, Rubella, Hepatitis B, Varicella
• Standard 9-panel drug test within 30 days of start
• COVID-19 vaccination or approved exemption
• Respirator medical clearance if respirator use is required
• Required facility-specific documents including photo for badge, personal/auto insurance, dress code & tobacco policy acknowledgment, and BayCare online learning modules completed prior to start
• References: 2 professional references from the past 2 years
• Employment application and skills checklist required

Additional Notes

• Scrubs: Any solid color scrub
• Documentation systems: BayCare Forms, BayCare Scheduling, Ace Timesheet
• Candidates must complete BayCare Online Learning Center modules within 48 hours of contract signing and prior to start date
• Mask wearing is mandatory if flu shot is declined
• Candidates must maintain all licenses and certifications current throughout the assignment
