Job Opportunity: Registered Nurse - Home Health

Location: Jackson, Tennessee
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Schedule: Monday to Friday, 8 AM to 5 PM.
• Patient visits include homes and assisted living facilities.
• Must be willing to travel up to 75 miles from home clinic; weekly travel could be up to 500 miles.
• Use EMR systems: HomeCare HomeBase, Oasis, Outlook.
• On-call requirement: Approximately 1 day per week (varies by location).
• Potential for full-time hire after contract completion.

Required Qualifications

• Specialty: Registered Nurse (Home Health)
• Experience: Minimum 1 full year of recent Adult Home Health experience or Hospice experience depending on specialty. First-time travelers are not accepted; recent specialty experience is required.
• Certifications: Current CPR and BLS certification required.
• Licensure: Must have an active RN license in hand (no pending licenses accepted).
• Travel: Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Technical Skills: Proficiency with HomeCare HomeBase (used within last 6 months) and Oasis required. Retraining may be required if not recently used.
• Dress Code: Scrubs.

Additional Requirements and Compliance

• Must complete a variety of background checks including but not limited to:

  • 7-year standard background check (including SS trace, county, nationwide, sex offender, state-specific searches).
  • EPLS/GSA/SAM and HHS/OIG checks.
  • Fingerprinting as required by state (varies by location).
• Education verification required.
• Drug screening: 10-panel drug screen required.
• Health documents required:

  • Physical exam within 12 months of start.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination for current season.
  • COVID vaccination series or documented declination.
• Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start.
• References: Two references required.
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.

Work Environment and Expectations

• Home health setting with patient visits in private homes and assisted living facilities.
• Travel required within a defined territory (up to 75 miles radius).
• Productivity expectations: Approximately 30 points per week (points assigned based on visits and activities).
• Schedule may vary by location; some positions may have weekend or on-call requirements.

This position is ideal for experienced Home Health RNs who are comfortable with travel, patient home visits, and working independently while maintaining compliance with agency and state requirements.
