Job Opportunity: Hospice Registered Nurse (RN)

Location: Katy, Texas

Contract Length: 13 weeks

Job Description

• Provide skilled nursing care to hospice patients in their homes.
• Perform comprehensive patient assessments and collaborate with the interdisciplinary team to develop and oversee individualized Plans of Care.
• Coordinate care with patients, families, and certifying providers.
• Use Homecare Homebase (HCHB) and OASIS for charting, which must be completed while at the patient’s home.
• Average caseload of 4-6 patients per day, approximately 30 points per week (40 hours/week).
• Participate in an on-call rotation, including one night of call during the week and weekends as needed.
• Must be willing to travel up to 50 miles outside the home base.

Required Qualifications

• Registered Nurse (RN) license valid in Texas (must have license in hand at time of assignment).
• Minimum 2 years clinical experience overall, with at least 1 year in Home Health or Hospice nursing.
• BLS certification required.
• Experience with HCHB and OASIS charting preferred (minimum 1 year preferred).
• Non-smoker preferred due to patient sensitivity.
• Must be open to on-call duties and weekend work as needed.
• First-time travelers accepted.
