Job Opportunity: Home Health Registered Nurse (RN)
Location: Portland, Oregon

Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Must be willing to travel up to 75 miles from home clinic; typical weekly travel is 20-40 miles.
- Schedule: Mostly Monday-Friday, 8 AM to 5 PM; some positions may have a Baylor shift schedule (4x10s, Friday-Monday).
- Weekend and on-call requirements vary; on-call expected 2-4 times per month.
Required Qualifications
- Specialty: Registered Nurse (Home Health)
- Experience: Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience depending on specialty. First-time travelers are not accepted; recent specialty experience is required.
- Certifications: Current CPR and BLS certification required.
- Licensure: Must have an active RN license in hand (no pending licenses accepted).
- Transportation: Reliable transportation and current driver’s license required. Agency-required liability insurance also required.
- EMR Proficiency: Must be proficient with HomeCare HomeBase (HCHB) and have used it within the last 6 months. RNs are also required to be proficient in OASIS; if not used within 6 months, retraining is required.
- Dress Code: Scrubs.

- Productivity expectations: 30 points per week (points assigned based on visits and activities).
- Locals are accepted at the same rate.
- Must be willing to comply with all required background checks, health screenings, and facility-specific documentation prior to start.
- Required health documents include physical exam, negative TB test or chest X-ray, immunizations (MMR, Hepatitis B, Varicella), drug testing, flu vaccination or declination, and COVID vaccination or declination.
- References: Two references required for all placements.
- Work history verification required for at least the last 7 years or two most recent employers.
