Job Opportunity: OT Home Health (Travel/Contract)

Location: Portland, Oregon (also covers Clark County, WA and surrounding areas)

Job Details

• Start Date: ASAP
• Shift: 8-hour day shifts (no 4x10 or 3x12 shifts available)
• Patient Ratio: 1:1 (up to 6 patients per day; 30 productivity points standard)
• Experience Required: Minimum 1 year of skilled home health experience
• First-Time Travelers: Accepted
• Weekend Work: Not required
• On-Call: Not required
• Floating: Not required
• Block Schedule: Not accommodated
• Locals: Accepted within 50-75 mile radius
• Pending License: Accepted with OR or WA licensure required
• Transportation: Traveler must have own transportation
• Travel Radius: Up to 60 miles per day (60-90 minutes total drive time)
• Productivity System: EPIC Remote Client for Home Health and Rover (phone app) / Oasis
• Attire: Business casual; scrubs allowed but not required (no specific color; no jeans)
• Parking: Free
• Guaranteed Hours: Facility may call off up to one shift every two weeks

Required Qualifications

• Valid Occupational Therapist license in Oregon
• Minimum 1 year of Occupational Therapist experience within the last year
• Experience in a Home Health setting
• BLS (Basic Life Support) certification

Compliance and Documentation Requirements

• Background Checks:

  • Must be completed within 45 days of start, covering all locations lived/worked in past 7 years
  • Includes SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (required for Home Health)
  • Oregon Abuse/Neglect search required only for specific facilities (not applicable here)
• State-Specific Checks:

  • Washington WATCH Search required for WA and border facility placements
  • Oregon Abuse Neglect Search not required for this placement
• Health Documents:

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit mask test (annual, travel only)
  • Immunizations/vaccinations: TDAP, MMR (Rubeola, Mumps, Rubella), Hepatitis B, Varicella, Flu seasonal vaccine (or declination), COVID vaccine or declination
  • Negative TB test or chest X-ray within 12 months, updated annually
  • 10-panel drug screen within 90 days of start (no instant tests)
  • Respiratory safety clearance (annual for travel)
• References:

  • Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting
• Employment Application:

  • Must include at least last 5 years of employment with month/year format
• Competency Exams:

  • Medication test/pharmacology competency as applicable
• Skills Checklist:

  • Required annually if applicable
• Modules and Training:

  • Completion of Providence WorkRamp modules including regulatory, specialty-specific, and regional core modules as assigned
  • EPIC training modules required prior to moving to active work status
• Other:

  • Traveler must complete float policy acknowledgement (though floating is not required for this role)
  • Must have a passport-style badge photo for ID

• Facility groups clients to minimize drive time.
• Locals within 50-75 miles are accepted at the same rate as travelers.
• No weekend or on-call shifts required.
• No floating between units required.
