Job Opportunity: OT Inpatient Rehab (Therapy/Rehabilitation)
Location: Medford, Oregon
Employment Type: Travel/Contract

Job Details
- Position floats between units
- No on-call required
- One regular weekend shift per week required
- Patient ratio: 6 patients per 8-hour day
- Facility type: Short Term Acute Care

Required Experience
- Specialty: Therapy/Rehabilitation (Occupational Therapy - OT Inpatient Rehab)
- Minimum years of experience: 1 year
- Prior travel experience: Not required (1st timers OK)
Required Licenses & Certifications
- Valid Occupational Therapy license for Oregon
- BLS (Basic Life Support) certification (AHA or Canadian Heart & Stroke Foundation)
- ACLS, PALS, NRP, Chemotherapy, EFM, NIH Stroke, CPI, IV Certification, MAB, and other certifications as applicable must be verified per clinical track (check specific requirements)
Required Documents & Compliance
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- 10 Panel Drug Screen within 90 days of start (no instant tests)
- Immunizations and health clearances:
- TDAP (within 10 years or declination accepted)
- Negative TB test or chest X-ray within 12 months (annual updates required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal Flu vaccine or declination (required during flu season)
- COVID vaccine or declination
- Respiratory Safety Clearance (RespSafety) required annually for travel candidates
- Background checks including nationwide, state-specific, and specialty-specific checks as applicable
- Employment application covering last 5 years (7 years for APN/Subcontractors)
- Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
- Skills checklists and competency exams as applicable, updated annually
Facility & Regional Requirements
- Must complete Providence Health and Services - Oregon WorkRamp modules prior to start (specialty and regulatory modules)
- Must comply with Providence float policy acknowledgment (may float to other Providence facilities nearby)
- Badge photo required (passport style with neutral background)
- Epic training modules required and must be completed prior to moving to productive work status
- Oregon-specific abuse/neglect search and other state-specific background checks as required
Work Schedule & Environment
- Short Term Acute Care setting
- Scrub color: Any for RN/Allied staff; scrubs provided for OR, Critical Care, ED, PACU, and Maternal Child units
- Parking: No fee
Requirements/Qualifications
- Valid Occupational Therapist license in Oregon
- Minimum of 1 year of Occupational Therapist experience
- Occupational Therapist experience within the last year
- Experience in an Inpatient setting
- Experience in a Rehabilitation setting
