Job Opportunity: Physical Therapist (Outpatient) - Travel/Contract

Location: Perry, Oklahoma
Contract Length: 13 weeks

Job Summary

• Provide Physical Therapy, Occupational Therapy, or Speech Therapy treatments for patients with disease or disability.
• Work independently in a small hospital setting.
• Provide preventive health measures.
• Offer technical direction to students, technicians, and therapy assistants.

Required Qualifications

• Valid Physical Therapist license in Oklahoma.
• Minimum 2 years of experience as a Physical Therapist.
• Physical Therapist experience within the last year.
• Experience as an Occupational Therapist.
• Experience as a Speech Language Pathologist.
• Bachelor of Science Degree from a physical therapy school, or a Bachelor's degree from a recognized Occupational Therapy program, or a Master's degree from a recognized Speech Therapy program.
• Basic Life Support (BLS) certification.

Additional Requirements

• Must complete a 7-year criminal background check prior to start.
• Drug screening (10-panel) within 30 days of start.
• Immunizations and health documentation required, including:

  • Tdap vaccine within 10 years (declination acceptable).
  • Negative TB test or chest X-ray within 12 months.
  • Measles, Mumps, Rubella, Varicella immunity proof (titers or vaccines).
  • Hepatitis B vaccination or immunity.
  • Flu vaccination or approved declination.
  • COVID vaccination or approved declination.
• Annual TB questionnaire.
• Professional references (2) within 3 years.
• Completion of required orientation and competency exams.
• Government-issued photo ID and valid state license verification.
• Completion of facility-specific orientation and compliance documents prior to start.

Certifications

• Basic Life Support (BLS) required.
• Advanced certifications (ACLS, PALS, NRP) not required for this role.

Notes

• Experience working independently in a small hospital setting is expected.
• Prior travel experience is not explicitly required.
• Electronic Medical Record (EMR) system experience preferred (Quick EMR mentioned).

This position is ideal for a licensed Physical Therapist with at least 2 years of experience seeking a travel contract in Oklahoma.
