Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Average patients per day: 5-8 (assigned 5-10 patients/day equating to roughly 6-8 points).
• Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
• Work schedule: Monday to Friday, 8 AM to 5 PM.
• No weekend rotation or on-call required.
• Must be willing to travel up to 75 miles from home clinic; average weekly travel 100-120 miles.
• Use of EMR system: HomeCare HomeBase (HCHB) and proficiency in OASIS required.

Required Qualifications

• Specialty: Registered Nurse (Home Health)
• Experience: Minimum 1 full year of Adult Home Health experience or 1 full year Hospice experience depending on specialty.
• First-timers: Not accepted; must have recent specialty experience.
• Certifications: Current CPR and BLS certification required.
• Licensure: Must have an active RN license in hand (no pending licenses accepted).
• Transportation: Reliable transportation and current driver's license required.
• Technology: Proficient with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last used over 6 months ago.
• Travel: Must be willing to travel within the assigned territory.

Additional Information

• Dress code: Scrubs.
• No guaranteed hours; patient census may vary.
• Local candidates accepted at the same rate.
• Some positions may have a Baylor shift schedule (4x10s Friday-Monday), but most are standard 5x8s Monday-Friday.

Compliance and Documentation Requirements

• Valid driver’s license and personal/auto insurance required.
• Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and fingerprinting as applicable by state.
• Education verification required.
• Health documents including physical exam (within 12 months), negative TB test or chest X-ray, immunizations (MMR, Hepatitis B, Varicella), drug screening, and COVID-19 vaccination or declination.
• Must complete various compliance trainings and sign confidentiality, privacy, and code of conduct agreements prior to start.
• References: Two professional references required.
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.
