Required Qualifications

  • License: Current RN license for Indiana with primary source verification within 30 days prior to assignment start
  • Certifications:
    • BLS (American Heart Association)

  • Ascension Skills Checklist (clinical, completed within 1 year of start, updated annually)

Health and Immunization Requirements

  • Two MMR vaccines or positive IGG titer (no declinations accepted)
  • Fit Test (preferred prior to start, can be completed onsite; updated annually)
  • Hepatitis B vaccination series or signed declination
  • Influenza vaccine (seasonal; exemptions accepted with mask requirement)
  • Positive IGRA or PPD results with negative chest X-ray within 1 year, plus annual TB questionnaire
  • Two doses of Varicella vaccine or serological evidence of immunity (no declinations accepted)
  • Tdap vaccine valid within 10 years (no declinations accepted)
  • Health statement or physical exam prior to first assignment, updated annually

Background and Verification Requirements

  • Nursys license verification within 30 days prior to start
  • License/Certificate primary source verification within 30 days prior to start
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and others within 30 days prior to start; updated annually or if break in service > 90 days
  • EPLS check within 90 days prior to start; rerun if break in service > 90 days
  • Education verification within 90 days prior to start; rerun if break in service > 90 days
  • Employment verification for past 7 years; updated annually or if break in service > 90 days
  • Resume/Profile submission required
  • At least two professional references prior to first assignment
  • Driver’s License/State ID matching home state for compact license (if applicable)

Drug Screening

  • 12 Panel Drug Screen within 30 days prior to start; includes amphetamines, cocaine, marijuana, PCP, methamphetamine, barbiturates, benzodiazepines, buprenorphine, methadone, oxycodone, morphine, and ecstasy
  • Must include MRO review for prescriptions or negative-dilute results

Additional Requirements

  • Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
  • Ascension Nursing Contract Worker Onboarding (completed onsite)
  • Ascension Confirmation of Assignment document
  • Ascension Badge Photo (headshot with white or blue background, no alterations)

This position requires a travel RN with medical/surgical experience to work nights on a pulmonary and medical overflow unit. The contract length is approximately 13 weeks. Candidates must meet extensive credentialing, immunization, and background verification requirements prior to assignment start.
