Job Description

• Work in Skilled Nursing and Rehab Units within a Long-Term Care (LTC) facility.
• Units function similarly to Med-Surg.
• Direct patient care with up to 20 residents per shift.
• Average daily census: 72 residents.
• Shift includes every other weekend rotation.
• No on-call required.
• No floating between units.
• Locals accepted.
• Block scheduling and floating not applicable.
• Attire: Any shade of blue scrubs.
• Charting system: PowerChart.
• Free parking available.

Requirements/Qualifications

• Valid RN license in Illinois.
• Associate of Science in Nursing / Associate Degree in Nursing.
• Minimum 1 year of Skilled Nursing experience within the last year.
• Rehabilitation experience.
• BLS (Basic Life Support) certification.
• ACLS (Advanced Cardiovascular Life Support) certification.
• IV Certification.

Special Requirements

• Excellent customer service skills.
• Experience working closely with CNAs/HCAs.
• Familiarity with special equipment such as lifting equipment.
• Rehab experience with dementia and surgical patient care, including IV medications and wound care.

Schedule

• Every other weekend required upon request of the center.
• Cannot take every holiday off.
• Any call-outs must be given with more than 24 hours' notice.

Background and Health Requirements

• Background checks required, including SS trace, county searches (7 years), nationwide sex offender, EPLS/GSA/SAM, and HHS/OIG.
• Physical exam within one year of start, updated annually.
• Immunizations/vaccinations required or documented declinations for:

  • Tetanus/Diphtheria/Tdap
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (declination accepted)
  • Varicella
  • Seasonal Flu vaccine or medical/religious exemption
  • COVID-19 vaccine (fully vaccinated required) or approved medical/religious exemption with testing requirements.
• Negative TB test or chest X-ray per facility policy.
• 9-panel drug screen within last 12 months (no pending drug screens allowed).
• Unit-specific competency exams and medication/pharmacology competency exam required.
• Two professional references within 2 years required.
• Employment application and unit-specific skills checklist required within 1 year of start.

Additional Notes

• Prior travel experience is NOT required.
• RTO (Request Time Off) only approved at time of submission; no approvals after offer.
• Facility does not accommodate every holiday off.
• Free parking; no mileage reimbursement.

This position is ideal for RNs with at least 1 year of experience interested in skilled nursing and rehab care in a LTC setting, including those new to travel nursing.
