Average Patients per Day: 6
Points per Day: 6.5 (Starts of Care 2.5, Discharges 1, Routine Visits 1, Recerts 1.75)
Guaranteed Hours: 40 per week

Territory Coverage: 75 mile radius from home clinic

Required Qualifications

Valid Physical Therapist license in Indiana
Minimum of 2 years of experience as a Physical Therapist
Physical Therapist experience within the last year

Additional Requirements

Charting Experience: Home Care Home Base (HCHB) preferred; training provided if needed
Dress Code: Scrubs
Pending License: Not accepted

Compliance and Documentation

Government Issued Photo ID required
Background Check: Standard 7-year search including SS Trace, County, Nationwide, Sex Offender, Motor Vehicle (updated annually)
EPLS/GSA/SAM and HHS/OIG checks required annually
Physical Exam within 1 year of start
Negative TB test or Chest X-Ray within 1 year (with specific policies for past positives)
Immunizations or Titers required for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
Drug Screening: 9 Panel Drug Screen within 1 year of start
Flu Vaccination or Declination required during flu season (Oct 1 - Mar 31)
COVID Vaccination or Declination required
Professional References: 2 for travel/allied/per diem acute care (within 3 years)
Skills Checklist: Required annually
Medication/Pharmacology Competency Exam as applicable

Notes

Candidates without recent HCHB experience (within 6 months) must complete self-paced online training and pass assessment before starting.
Mileage reimbursement at standard rate.
Locals within the 75-mile radius are accepted.

This position is ideal for experienced Physical Therapists with at least 2 years in the specialty, comfortable with home health care, and able to commit to a 13-week travel contract in Indiana.
