Job Opportunity: Physical Therapist (Home Health) - Travel/Contract

Location: Fort Wayne, Indiana

Job Details

  • Average Patients per Day: 6
  • Points per Day: 6.5 (Starts of Care 2.5, Discharges 1, Routine Visits 1, Recerts 1.75)
  • Guaranteed Hours: 40 per week
  • Types of Cases: Starts of Care, Recerts, Discharges, Routine Visits
  • Territory Coverage: 75 mile radius from home clinic
  • Locals Accepted: Yes
  • Mileage Reimbursed

Required Qualifications

  • Valid Physical Therapist license in Indiana
  • Minimum of 2 years of experience as a Physical Therapist
  • Physical Therapist experience within the last year

Additional Requirements

  • Charting Experience: Home Care Home Base (HCHB) preferred; training provided if needed
  • Dress Code: Scrubs
  • Pending License: Not accepted

Compliance and Documentation

  • Government Issued Photo ID required
  • Background Check: Standard 7-year search including SS Trace, County, Nationwide, Sex Offender, Motor Vehicle (updated annually)
  • EPLS/GSA/SAM and HHS/OIG checks required annually
  • Physical Exam within 1 year of start
  • Negative TB test or Chest X-Ray within 1 year (with specific policies for past positives)
  • Immunizations or Titers required for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug Screening: 9 Panel Drug Screen within 1 year of start
  • Flu Vaccination or Declination required during flu season (Oct 1 - Mar 31)
  • COVID Vaccination or Declination required
  • Professional References: 2 for travel/allied/per diem acute care (within 3 years)
  • Skills Checklist: Required annually
  • Medication/Pharmacology Competency Exam as applicable

Notes

  • Candidates without recent HCHB experience (within 6 months) must complete self-paced online training and pass assessment before starting.
  • Mileage reimbursement at standard rate.
  • Locals within the 75-mile radius are accepted.

This position is ideal for experienced Physical Therapists with at least 2 years in the specialty, comfortable with home health care, and able to commit to a 13-week travel contract in Indiana.
