Required Qualifications & Certifications

• Valid RN license in Washington
• Wound Care experience within the last year (minimum 1 year)

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Fit mask test (N95 or equivalent) valid within 1 year, updated annually
• Immunizations/vaccinations:

  • Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
  • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer or declination)
  • Hepatitis B vaccination or signed declination
  • Varicella (2 vaccines or positive titer or declination)
  • Seasonal Flu vaccine or declination (required during flu season)
  • COVID-19 vaccine or declination
• Negative TB test or chest X-ray within 12 months, updated annually
• Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
• Respiratory Safety Clearance (RespSafety) questionnaire and clearance annually for respirator use

Background & Compliance

• Background check within 45 days of start covering all locations lived/worked in past 7 years
• Education verification required (nursing degree verification)
• Employment application covering last 5 years (7 years for APN/Subcontractors)
• Minimum 2 references from supervisors covering 1 year of employment in comparable specialty/setting within last 3 years
• State-specific background checks and registry searches as required by Washington state law
• Completion of Providence-specific WorkRamp training modules annually, including:

  • Regulatory requirements
  • Specialty-specific modules for WOC RN role
  • Basics of Trauma Informed Care
• Epic training modules completion required prior to moving to paid status
