Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Job Description

• Provide physical therapy services in a home health setting, including home visits and periodic office meetings.
• Average patients per day: 5-6 (assigned between 5-10 patients daily).
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM, with 1 weekend (Saturday and Sunday) required per month.
• Work involves visiting patients both in their homes and care facilities.
• Must be willing to travel up to 75 miles from the home clinic.
• Use of EMR system: Home Care Home Base.
• On-call requirement: 1 weekend per month (varies, discuss during interview).
• No guaranteed hours; if patient census is low, may be floated to a nearby clinic to pick up hours.
• Local candidates accepted.

Required Qualifications

• Valid Physical Therapist license in Missouri.
• Minimum of 1 year of recent experience as a Physical Therapist.
• Experience working with adult patients in a Home Health setting.
• Current BLS certification.
• Reliable transportation and current driver's license.
• Agency-required liability insurance.
• Must be willing to travel extensively within the assigned territory.

Productivity Expectations

• Weekly productivity units: 30 points.
• Travel mileage reimbursed at IRS standard rate (compensated from patient home to patient home; commute to/from first and last patient is not billable).
• Expected travel: approximately 30-40 miles per day, totaling about 100 miles per week.

Additional Requirements & Compliance

• Background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as required.
• Fingerprinting as required by state regulations.
• Education verification.
• Drug screening (10-panel).
• Immunizations and health documents including:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination.
  • COVID-19 vaccination or documented declination.
• Completion of compliance training and review of policies including HIPAA, confidentiality, and home health/hospice specific scenarios.
• References: Two professional references required.
• Work history verification for the last 7 years or at least two most recent employers.
