Job Opportunity: Physical Therapist - Home Health

Location: Shelton, Connecticut

Job Description

• Provide physical therapy services in a home health setting with home visits.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Typically Monday to Friday, 8 AM to 5 PM; some positions may have alternate schedules such as 4x10 shifts (Friday-Monday).
• Mileage reimbursement provided for travel between patient homes (commute to/from first and last patient not reimbursed).
• Must be willing to travel up to 75 miles from home clinic, with weekly travel estimated between 200-300 miles.
• Use of electronic medical records system (HCHB).
• Local candidates accepted.

Required Qualifications

• Valid Physical Therapist license in Connecticut.
• Minimum of 1 year of Physical Therapist experience within the last year.
• Experience working with adult patients.
• Experience in a home health setting.
• BLS certification.

Additional Information

• Weekend and on-call requirements vary by location and will be discussed during the interview.
• Two professional references required.
• Verification of work history for the last 7 years or at least two most recent employers.
• Must complete all required background checks including state and federal screenings.
• Candidates must complete compliance training and review facility-specific policies prior to assignment start.
• Some state-specific forms and attestations may be required depending on placement location.
