Registered Nurse - Skilled Nursing and Rehab Unit

Location: Belleville, Illinois
Employment Type: Travel/Contract
Contract Length: 16 weeks


Job Overview

  • Work in skilled nursing and rehab units within a long-term care (LTC) facility, functioning similarly to a Med-Surg environment.
  • Direct patient care with ratios up to 20 residents per shift; average daily census is 72.
  • Shift includes every other weekend rotation.
  • Start date: ASAP.

Required Qualifications

  • Valid Registered Nurse (RN) license in Illinois.
  • Minimum 2 years of skilled nursing experience, with at least some experience within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • IV Certification.
  • BLS (Basic Life Support) certification.
  • ACLS (Advanced Cardiovascular Life Support) certification.

Work Environment & Additional Details

  • Collaborate closely with CNAs and HCAs.
  • Use of special lifting equipment.
  • Facility provides specialized medical and rehabilitation care to dementia and surgical patients, including IV medications and wound care.
  • Attire: Any shade of blue scrubs.
  • Free parking available.
  • No mileage reimbursement.

Compliance & Documentation Requirements

  • Background checks including SS trace, county searches (7 years), nationwide sex offender, EPLS/GSA/SAM, and HHS/OIG required.
  • Professional license primary source verification required.
  • Unit-specific competency exam and medication/pharmacology competency exam required.
  • Physical exam within one year of start, updated annually.
  • Immunizations and health documents required:
    • BLS/ACLS certifications
    • Tetanus/Diphtheria/Tdap (vaccine or declination)
    • Negative TB test or chest X-ray (per guidelines)
    • Measles, Mumps, Rubella (positive titer or vaccination)
    • Hepatitis B declination accepted
    • Varicella immunity or vaccination
    • Seasonal flu vaccine or medical/religious exemption
    • COVID-19 vaccination required unless medically or religiously exempt (with testing requirements for exemptions)
  • 9-panel drug screen within last 12 months (instant tests accepted; cannot start with pending drug screen).
  • Two professional references within the last 2 years.
  • Employment application and unit-specific skills checklist required within 1 year of start.
  • Photo for badge required prior to start.

Additional Notes

  • Any call-outs must be given with more than 24 hours' notice.
  • Requests for time off (RTO) must be approved at time of submission; not approved after offer.
  • Cannot guarantee every holiday off.
  • Pending licenses accepted only with proof of license.
  • No floating between units required.
  • Block scheduling accommodations not applicable.

This position offers an opportunity to work in a skilled nursing and rehab setting with a focus on dementia and surgical patient care, requiring strong clinical skills and excellent teamwork.
