Location: Portland, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking an Occupational Therapist for home health services in West Portland and surrounding areas. This role involves providing skilled home health therapy with a typical 8-hour day shift schedule. The position requires traveling to patient homes within a radius of up to approximately 60 miles per day.
Key Details
- Shift: 8-hour day shifts (no 4x10 or 3x12 shifts available)
- Patient Load: 1:1 ratio; may visit up to 6 patients per day
- Start Date: ASAP
- Weekend Requirement: No
- On-Call: No
- Floating: No floating between units required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Transportation: Must have own transportation
- COVID-19 Vaccination: Primary series plus one booster required
- Charting System: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation required
- Attire: Business casual; scrubs allowed but not required (no jeans)
- Parking: Free
- Mileage Reimbursement: IRS standard rates
Required Qualifications
- Valid Occupational Therapist license in Oregon
- Minimum 1 year of Occupational Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Experience with OASIS documentation
- Basic Life Support (BLS) certification
Special Requirements
- Must have reliable personal transportation
- Ability to travel up to 60 miles per day (approximately 60-90 minutes total drive time)
- Facility may call off up to one shift every two weeks (guaranteed hours with some flexibility)
Compliance and Documentation Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Drug screening (10-panel) within 90 days of start
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health documents including:
- COVID-19 vaccination or declination
- Flu vaccine or declination (seasonal)
- Tetanus/Diphtheria/Tdap within 10 years
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Varicella vaccination or positive titer
- Hepatitis B vaccination or declination
- Respirator clearance required annually for travel candidates
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application covering at least last 5 years
Training and Competency
- Completion of required WorkRamp modules prior to start (annual regulatory and specialty-specific modules)
- Competency exams as applicable by profession
- Medication test/pharmacology competency exam as applicable
- Facility-specific float policy acknowledgment
Notes
- This position does not require weekend or on-call shifts.
- The facility does not accommodate block schedules or floating between units.
- First-time travel therapists are welcome to apply.
- The role requires adherence to strict vaccination and health screening policies.
- Documentation and compliance must be maintained and updated as per facility and state requirements.
