Required Experience

• Minimum 2 years ICU experience required
• First-time travelers accepted only if they have 2 years ICU experience
• Experience with 1:2 patient ratios required
• Experience in interpretation and management of dysrhythmias required
• IV therapy experience required
• Neurological experience required
• Respiratory/Thoracic experience required

Required Certifications & Licensure

• Compact or Guam RN license required at time of application (no pending licenses accepted)
• ACLS (American Heart Association provider card required; instructor cards not accepted)
• BLS (American Heart Association provider card required)
• NIH Stroke Certification (preferred)
• Conscious Sedation certification preferred
• TCRN preferred but not required
• Additional certifications may be required depending on unit (e.g., PALS, NRP for certain units)

Work Requirements

• Weekend shifts required (minimum every other weekend, based on unit needs)
• Holidays required (dates determined by unit; no holiday shift requests allowed)
• On-call: No
• Floating: Yes, within scope as needed by unit
• Return to Office (RTO) restrictions: Must be approved and included in contract prior to start; max 3 consecutive days and 7 total days

Special Requirements

• Passport in hand required (US passport only; Canadian passports not accepted)
• Must be willing to work holidays and weekends as scheduled
• Must be able to interpret/manage dysrhythmias and provide IV therapy
• Must have neurological and respiratory/thoracic experience

Orientation and Testing

• Drug Calculations and Basic Knowledge Assessment Tool (BKAT) testing onsite during orientation; must pass with 80% or higher; two attempts allowed
• Competency exams required as applicable

Health and Compliance Requirements

• Physical exam within 1 year (facility does not require but agency does)
• Negative TB test or chest X-ray within 1 year; annual TB questionnaire if past positive
• Fit test for approved masks within 1 year; may be done onsite if needed
• 9-panel drug screen within 30 days of start; instant drug screen acceptable
• Required immunizations or proof of immunity: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella
• Flu vaccine mandatory during flu season (Sept 1 - June 30) or approved medical exemption required
• COVID vaccination strongly preferred; declination requires facility-specific waiver

Required Training Modules (must be completed by compliance deadline)

• Pressure Injury Training (NPIAP)
• NIH Stroke Scale Training (NIHSS)
• Alaris IV Pumps
• Pyxis CareFusion Medication System
• Conscious Sedation (required for ICU, ER, Cath Lab)
• Emergency Severity Index (ESI) training for ER staff
• FEMA modules completed onsite

Additional Notes

• Candidates should plan to arrive at least one day before start due to long travel times and time zone differences
• Parking is free for travelers; registration with HR required
• Dress code: Navy blue preferred; any solid color scrub acceptable; jackets of any color allowed
• Charting system: Allscripts/Sunrise

This position is ideal for experienced ICU RNs with at least 2 years of critical care experience who are willing to work nights, weekends, and holidays in an acute care trauma setting in Guam. Candidates must have all required licenses and certifications in hand prior to application and be prepared for onsite competency testing and mandatory training modules.
