Work Requirements
- On-call: No
- Floating: Yes, within scope as needed by unit
- Weekend Requirement: Minimum every other weekend, based on unit needs
- Holidays: Required to work holidays as scheduled by unit; no holiday work requests accepted
- Locals: Not accepted (candidates residing in Guam are considered local and will not be considered)
- Guaranteed Hours: Contracted hours
- Return to Office (RTO) Restrictions: Must be approved and included in contract prior to start; max 3 consecutive days and 7 total days
Experience Requirements
- Minimum 2 years ICU experience required
- First-time travelers accepted only if they have 2 years ICU experience
- Experience with 1:2 patient ratios required
- Required clinical experience includes:
- Interpretation and management of dysrhythmias
- IV therapy
- Neurological experience
- Respiratory/Thoracic experience
Licensure & Certifications
- Must have Compact or Guam RN license at time of application (no pending licenses accepted)
- Required Certifications:
- ACLS (Advanced Cardiac Life Support)
- BLS (Basic Life Support)
- NIH Stroke Certification
- Conscious Sedation
- Preferred Certification:
- TCRN (Trauma Certified Registered Nurse)
- Additional certifications may be required depending on unit (e.g., PALS, NRP for ER)
- All certifications must be current; candidates with expiring certifications within 6 months may need to renew before start (onsite renewal may be available)
Special Requirements
- Passport in hand required for travel
- Experience working with 1:2 patient ratios
- Must be able to interpret/manage dysrhythmias, provide IV therapy, and have neurological and respiratory/thoracic experience
Orientation and Testing
- Onsite drug calculations and Basic Knowledge Assessment Tool (BKAT) testing required during orientation
- Tests are pen and paper, must be completed in person on the first day
- Passing score: 80% or higher; two attempts allowed with remediation discussion after first attempt
Health and Compliance Requirements
- Physical exam within 1 year (not required by facility but required by agency)
- Fit mask test within 1 year prior to start; must use approved masks or complete fit test onsite
- Negative TB test or chest X-ray within 1 year; annual TB questionnaire if past positive
- Immunizations or proof of immunity required for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 30 days of start (instant drug screen acceptable)
- Flu vaccine mandatory during flu season (Sept 1 - June 30); medical exemptions considered with proper documentation
- COVID vaccination strongly preferred; declination requires facility-specific waiver form
Facility-Specific Training Modules (must be completed by compliance deadline)
- Pressure Injury Training (NDNQI)
- NIH Stroke Scale Training (NIHSS)
- Alaris IV Pumps
- Pyxis CareFusion Medication System
- Conscious Sedation (required for ICU, ER, Cath Lab)
- Emergency Severity Index (ESI) training for ER staff
- FEMA modules completed onsite
Additional Notes
- Candidates should plan to arrive at least one day before start due to long travel times and time zone differences
- Parking is free for travelers; registration with HR required
- Scrub color: Navy blue preferred; solid colors acceptable; jackets of any color allowed
- Charting system: Allscripts/Sunrise
References and Work History
- 2 professional references required for travel, allied, and per diem acute care roles
- Work history verification for last 10 years required; attestations accepted if verification is not possible
This position is ideal for experienced ICU RNs with at least 2 years of critical care experience who are willing to work nights, weekends, and holidays in a trauma-level acute care setting in Guam. Candidates must be fully licensed and certified, able to complete required facility-specific training, and prepared for travel logistics including passport and health compliance.
