RN Case Manager - Home Health/Hospice
Location: Lebanon, New Hampshire
Employment Type: Contract
Contract Length: 16 weeks
Job Description
As an RN Case Manager in Home Health/Hospice, you will:
- Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
- Coordinate care plans through interdisciplinary communication and case conferences.
- Provide ongoing assessment of patients’ clinical, functional, and emotional status, communicating significant changes to physicians and team members.
- Document evaluations, treatment goals, and objectives.
- Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.
This role offers broad independence while being part of a cohesive patient-care team, with a focus on work/life balance and flexibility.
Required Qualifications
- Licensed Registered Nurse (RN) in Vermont and New Hampshire or hold an unencumbered compact license.
- Graduate of an accredited school of nursing.
- Minimum 2 years of experience in Home Health and Hospice nursing.
Additional Information
- Full-time, day shifts.
- Prior travel experience is not specified as required.
