Job Opportunity: Certified Occupational Therapy Assistant (COTA)

Location: Tulsa, Oklahoma
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Days, 5 days a week, 8-hour shifts (8:00 AM - 4:30 PM or 9:00 AM - 5:30 PM)
  • Charting System: Epic (charting experience must be noted on profile)
  • Weekend Requirement: Potentially 1 weekend per month
  • Block Schedule: Accommodations possible
  • Locals Accepted: Yes
  • Time Off Restrictions:
    • No holiday time off
    • Maximum 5 total days off during the 13-week assignment
    • No time off requests after contract submission or while working
    • No time off day before, day of, or day after a holiday

Required Experience & Qualifications

  • Valid Certified Occupational Therapy Assistant license in Oklahoma
  • Certified Occupational Therapy Assistant with at least 1 year of experience within the last year
  • One completed travel assignment
  • BLS (Basic Life Support) certification
  • Experience in an Acute Care setting
  • Experience in an Outpatient setting

Patient Population / Common Diagnoses

  • Stroke (CVA)
  • Coronary Artery Bypass Graft (CABG) and other thoracic/cardiac surgeries
  • Orthopedic surgeries
  • Respiratory failure
  • Chronic conditions such as COPD
  • Non-healing wound management
  • Amputations

Facility Type

  • Acute Care Hospital
  • Level 3 Trauma Center
  • 171 staffed beds

Policies & Requirements

  • Attendance: Zero-fault attendance policy; any missed shift counts as unexcused. Two missed shifts result in a warning; three or more may result in cancellation.
  • Background Check: Complete 7-year criminal background check required within 30 days of start (including multiple county and nationwide searches). Instant statewide searches are not accepted.
  • Education Verification: Copy of diploma or transcript required (unofficial transcripts acceptable if graduation date is listed). Education verification required if transcript/diploma cannot be provided.
  • Competency Exam: Unit-specific competency exam must be submitted.
  • Modules: Completion of required facility-specific training modules before start date.
  • Photo for Badge: Must provide a clear, front-facing photo with a plain white background before start.
  • Health Documents:
    • Physical exam within one year stating "cleared to work" with physician signature
    • Negative TB test or chest X-ray within one year (or chest X-ray within 5 years if prior positive)
    • Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
    • Drug test within 30 days of start (10-panel, negative results)
    • Flu vaccination or approved exemption required during flu season (Oct 1 - May 1)
  • References: Two references required prior to start
  • Work History: Verification of last 3 years required (attestations acceptable; can be pending at start)
