Schedule

• Monday to Friday, no weekends
• Holidays: May be expected to work 2 of 3 major holidays
• No call requirements
• Floating: May cross county lines if patient care requires

Job Description

• Position in Pre-Screen Unit; clinical fit will be reviewed before final manager interview
• May care for COVID-19 patients
• Tobacco and fragrance-free facility

Required Experience & Qualifications

• Valid Physical Therapist license in Pennsylvania
• Minimum 2 years of Physical Therapist experience, including experience within the last year
• Experience with Epic electronic health record system

Required Certifications

• PT License (PA or Compact)
• Basic Life Support (BLS) and other certifications as applicable (refer to unit/job specifics)

Required Documentation & Compliance

• Background checks including 7-year criminal history, EPLS/GSA/SAM, HHS/OIG
• PA FBI Fingerprinting within 1 year of start; results must be on file within 45 days of assignment start
• Pennsylvania Child Abuse Search within 5 years; results must be on file within 45 days of assignment start
• Pennsylvania State Police PATCH Search (must be finalized prior to start)
• Education verification of highest degree/diploma/transcripts
• Physical exam within 90 days of start, signed by MD, DO, PA, or NPT
• Immunizations and titers required:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test (QGold or TSpot) within 90 days of start (PPD not accepted)
  • Rubeola (Measles), Mumps, Rubella: 2 vaccines or positive titer with booster requirements
  • Hepatitis B: Positive titer or full vaccine series (3 doses) plus declination form if applicable
  • Varicella: 2 vaccines or positive titer with booster requirements
  • Seasonal Flu vaccination or valid medical/religious exemption required during flu season
  • COVID-19 vaccine or valid exemption/declination accepted
• Drug testing: Negative 9-panel within 1 year (for agency use only)
• Respiratory Safety clearance within 1 year, renewed annually

Additional Requirements

• Two professional references within 3 years
• Most recent resume or employment application
• Annual skills checklist
• Photo for badge (passport-style, professional appearance)
• Completion of facility-specific documents including code of conduct, confidentiality agreement, and health & wellness commitments
• Completion of required Learning Management System (LMS) modules and new hire orientation

Onboarding & Orientation

• New hire orientation must be completed by the end of the first day
• LMS modules must be completed on PC/laptop prior to start date

Notes

• Maximum return to office (RTO) time: 7 days
• No weekend shifts required
• Candidate may be required to work some major holidays
• Facility is tobacco and fragrance free; candidates should be aware prior to submission
