Job Description

Seeking a Physical Therapist with outpatient experience to provide therapy and rehabilitation services. The position follows a 100-mile radius rule for mileage reimbursement, which is reviewed on a case-by-case basis.

Required Qualifications

• Physical Therapist with a minimum of 1 year of experience
• Experience as a Physical Therapist within the last year
• Minimum of 1 year of outpatient physical therapy experience
• Basic Life Support (BLS) certification required

Compliance and Documentation Requirements

• Proof of identity and eligibility to work (I9, government-issued photo ID)
• Background checks including 7-year criminal history, sex offender registry, and FACIS Level III screening
• Education verification or proof of highest degree earned
• Physical exam within 60 days of start, confirming ability to perform essential job functions
• N95 Fit Test within 12 months of start (onsite testing available if needed)
• Immunizations and health screenings including:

  • Negative TB test or chest X-ray within 12 months
  • Measles, Mumps, Rubella (MMR) immunity or vaccination
  • Hepatitis B immunity or declination form
  • Varicella immunity or vaccination
  • Seasonal flu vaccine or declination during flu season
  • COVID-19 vaccine or declination accepted
• Drug screening: 9 or 10 panel drug screen within required timeframe
• Completion of facility-specific health and safety modules prior to start (approximately 8 hours)
• Professional references within 2 years (2 required for travel positions)
• Employment application and skills checklist as applicable

Additional Notes

• Facility requires primary source verification of state license
• Background checks and certain screenings must be updated annually or with each contract renewal
• Facility does not accept instant background checks as placeholders; full checks must be on file before start
• Healthstream modules and other orientation materials will be assigned after contract signing and must be completed before starting
• Physical exam must be signed by an MD, DO, PA, or ARNP
• If candidate has a history of positive TB test, additional documentation is required
