Job Opportunity: Physical Therapist

Location: Pensacola, Florida
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Details

• Full-time, 40 hours per week
• Every 4th weekend rotation required
• Holiday shifts required if they fall on a regularly scheduled shift

Required Qualifications

• Valid Physical Therapist license in Florida
• Minimum of 1 year of Physical Therapist experience, including experience within the last year
• Florida Licensed Physical Therapist

Required Credentials and Compliance

Candidates must provide and maintain the following credentials and health documentation:

Professional Credentials (required for submission or onboarding)

• Valid Florida Physical Therapist license
• Driver’s License or State ID (must not be expired and address must match licensing requirements)
• Professional resume/profile
• At least two professional references
• Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and other relevant checks (updated annually or after breaks in service)
• Employment verification for the past seven years (updated annually or after breaks in service)
• Education verification relevant to clinical work (must be current within 90 days of start)
• Signed assignment agreement and confirmation of assignment documentation
• Badge photo (forward-facing headshot with white or blue background, no alterations)
• Skills checklist for Physical Therapist specialty (to be completed within one year of start and updated annually)

Personal Health History Requirements (required for onboarding)

• MMR: Documented proof of two vaccines or positive immunity titer (no declinations accepted)
• Fit Test: Required for all travelers; preferred before start but can be completed onsite
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Tuberculosis Screening: 2-step PPD or IGRA test with annual TB questionnaire; if positive, chest X-ray within one year required
• Varicella: Proof of immunity or two vaccine doses (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
• 12 Panel Drug Screen: Within 30 days prior to start and updated annually; retesting required after breaks in service

Additional Notes

• All credentials and health requirements must be current and compliant prior to or at onboarding.
• Candidates should be prepared for weekend rotations and holiday work as specified.
