Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must be willing to travel up to 75 miles from the home clinic, with an expected weekly travel of 200-400 miles.
• Work schedule is typically Monday through Friday, 8 AM to 5 PM.
• Weekend and on-call requirements vary; minimal on-call backup Monday through Thursday.
• Use of electronic medical records systems: HomeCare HomeBase (HCHB) and OASIS proficiency required.
• Dress code: Scrubs.

Required Qualifications

• Valid Registered Nurse (RN) license in New Hampshire.
• Minimum of 1 year of hospice experience within the last year.
• Experience working with adult patients in a home health or hospice setting.
• Proficiency with OASIS documentation.
• Current BLS certification issued by the American Heart Association or American Red Cross.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Experience with Home Health care.

Additional Information

• No guaranteed hours; patient census may fluctuate.
• Candidates with both home health and hospice experience preferred, but hospice-only experience is acceptable.
• Weekend and on-call requirements vary by location and should be discussed during the interview.
• Local candidates are accepted.
• Must be able to meet all background checks, health screenings, and compliance requirements prior to start.
• Extensive background checks and state-specific documentation required.
• Candidates must complete various compliance and policy reviews prior to assignment start.
• Physical exam, immunizations, drug screening, and other health documents must be current and on file.
