Registered Nurse - Home Health (Florida, Celebration)

• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Schedule: Monday to Friday, 8:00 AM – 5:00 PM
• Work Setting: Patient homes and assisted living facilities
• Average Patients per Day: 5-8
• Travel Requirements: Must be willing to travel up to 75 miles from home clinic; approximately 150-200 miles per week
• EMR Systems: HomeCare HomeBase (HCHB), OASIS, Outlook
• On-Call: No on-call required, but may assist on weekends if needed
• Dress Code: Scrubs

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, current driver’s license, and agency-required liability insurance.
• Productivity expectations: 25-30 weekly units (30 points per week system).
• Charting proficiency required in HomeCare HomeBase and OASIS (RNs must be proficient in OASIS).
• Weekend requirements and on-call expectations vary; discuss during interview.

Required Qualifications

• Valid RN license in Florida
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 1 year of recent Home Health nursing experience, including experience with adult patients
• Minimum 1 year of recent Hospice nursing experience (depending on specialty)
• Current BLS certification issued by the American Heart Association or American Red Cross
• Proficiency with HomeCare HomeBase EMR system
• Experience working in a Home Health setting within the last year
