Registered Nurse - Home Health

Job Description

The Registered Nurse provides skilled nursing care to patients in their homes, coordinating care with patients, families, and interdisciplinary teams. Responsibilities include comprehensive patient assessments, certifying providers, and developing and overseeing individualized Plans of Care. The role involves in-home patient care, interdisciplinary care coordination, and working primarily with a geriatric population.

• Work schedule: Monday to Friday, 8 AM to 5 PM, with on-call rotation (including one night of call during the week and weekends as needed)
• Average patient load: 4-6 patients per day
• Charting: Must complete documentation using Homecare Homebase (HCHB) and OASIS while at the patient’s home
• Must be willing to travel up to 50 miles from home base
• Non-smokers preferred due to patient sensitivity

Required Qualifications

• Valid Registered Nurse (RN) license in Texas
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 2 years of clinical experience as a clinician
• At least 1 year of Home Health nursing experience within the last year
• BLS certification issued by the American Heart Association or American Red Cross
• Experience working in a Home Health setting

Additional Requirements

• Must be a non-smoker due to patient population sensitivity
• Must complete background checks including seven years of residency/work history, drug screening, and other standard verifications
• Required health documents include physical exam, TB testing/questionnaire, immunization proof (Measles, Mumps, Rubella, Hepatitis B, Varicella), and flu vaccination or declination during flu season
• Must provide professional references and complete an employment application
• Annual skills checklist and competency exams required

• Mileage reimbursement follows IRS standard rates
• Local candidates within a 50-mile radius are accepted
• Pending licenses are not accepted; license must be current and valid at start
