Job Details

Required Qualifications

• Valid Speech Language Pathologist license in Oregon

Experience

• Background Check within 45 days of start, covering all locations lived/worked in past 7 years

• Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
• Drug Screen: 10 panel within 90 days of start (no instant tests)
• Immunizations and Health Documents:

  • TDAP (within 10 years) or declination
  • Negative TB test or chest X-ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
  • Hepatitis B vaccination series or signed declination
  • Varicella vaccination or positive titer or declination
  • Seasonal Flu vaccine or declination (required during flu season)
  • COVID vaccine or declination
• Respiratory Safety Clearance (RespSafety) required annually for travel candidates
• Fit Mask testing required annually (specific mask types accepted)

Training & Competency

• Completion of required WorkRamp modules prior to start, including:

  • System-level regulatory modules (annual)
  • Specialty-specific modules for inpatient rehab or therapy roles
• Epic training modules completion required within a week of orientation
• Medication/Pharmacology competency exam as applicable
• Skills checklists updated annually as applicable

• Float Policy Acknowledgement required (may be required to float to other nearby facilities)
• Passport-style badge photo required prior to start
• Background checks and state-specific searches as required by Oregon and Washington regulations
