Registered Nurse - Home Health

Location: Indianapolis, Indiana
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Average patient load: 5-8 patients per day.
• Types of visits: Start of Care, Wound Care, Follow-up Visits, etc.
• Must be willing to travel up to 75 miles from home clinic, averaging 150-200 miles per week.
• Schedule: Monday to Friday, 8 AM to 5 PM.
• Weekend rotation: Every 3-4 weeks.
• On-call: Every 3-4 weeks.
• Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
• Dress code: Scrubs.

Required Qualifications

• Specialty: Registered Nurse (RN) in Home Health.
• Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience depending on specialty.
• First-time travelers not accepted; must have recent specialty experience.
• Current, valid RN license in hand (no pending licenses accepted).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver's license required.
• Must have agency-required liability insurance.
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last used over 6 months ago.
• RNs must be proficient in OASIS documentation.

Additional Information

• Must be able to complete various background checks, verifications, and state-specific requirements prior to start.
• Physical exam within 12 months required, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months required.
• Immunization or titer proof required for Measles, Mumps, Rubella, Hepatitis B, Varicella.
• Drug screening (10-panel) required.
• COVID vaccination series or declination required.
• Two professional references required.
• Work history verification for at least the last 7 years or two most recent employers.
• Candidates must review and sign various compliance, confidentiality, and policy documents prior to start.

Travel Experience

• Prior travel experience not explicitly required, but recent specialty experience is mandatory.
