Required Certifications

• BLS (Basic Life Support): Required for all candidates (AHA provider or AHA RQI accepted; RQI provider from other vendors not accepted)
• CPI (Crisis Prevention Institute): Required for Behavioral Health clinical candidates (may be completed onsite twice per month)

Other certifications such as ACLS, PALS, NRP, TNCC, EFM may be required or recommended for specific units but are not mandatory for this role.

Required Licensure

• Must hold a valid Registered Nurse (RN) license in Illinois
• Primary source verification of license must be completed within 30 days of assignment start
• NURSYS verification required within 30 days of assignment start

Requirements/Qualifications

• Valid Registered Nurse (RN) license in Illinois
• Minimum of 1 year of Behavioral Health nursing experience within the last year
• BLS certification issued by the American Heart Association
• CPI certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Required Documents & Checks

• Valid Driver’s License or State ID
• Background checks including 7-year history, aliases, sex offender, national and federal criminal checks, education verification, and others as specified
• Education verification (ADN or BSN required)
• Physical exam within 1 year, updated annually
• Fit mask test within 1 year (specific mask models required)
• Immunizations and titers (must be current or have valid exemptions):

  • Tetanus/Diphtheria/Tdap
  • Negative TB test or chest X-ray (within 1 year)
  • Rubeola (Measles), Mumps, Rubella
  • Hepatitis B (vaccination or immunity proof)
  • Varicella
  • Seasonal Flu vaccination or approved exemption (religious or medical)
• Drug testing: Negative 9-panel within 30 days of start
• Fingerprinting and Illinois Department of Public Health registry (if applicable)
• Employment application and verification of last 7 years of work history
• Skills checklist updated annually for assigned and float units
• Two professional references

Additional Information

• Orientation modules required before start (approx. 2 hours)
• COVID-19 and Influenza vaccine religious and medical exemption forms available; must be submitted and approved prior to start
• Candidates must comply with all facility-specific policies and documentation requirements
