Job Description

Seeking a Relief Pharmacist to cover recruiting, hiring, training, and to provide coverage for someone on leave. The position requires working 40 hours per week.

Required Qualifications

• Valid Clinical Pharmacist license in Oregon
• Minimum of 1 year of Clinical Pharmacist experience, including experience within the last year

Background and Compliance Requirements

• 7-year criminal background check required within 90 days of start; updated annually. Includes SSN trace, counties lived/worked/school, FACIS L3, DOJ sex offender search, nationwide checks, and education verification.
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start.
• Diploma or transcript must be on file.
• Primary source verification of state license required.
• Medication test/pharmacology competency exam as applicable.
• Photo for badge required within 48 hours of signing.
• Completion of facility-specific confidentiality, compliance, privacy, emergency contact, and contractual agreements required within 48 hours of signing.

Health and Safety Requirements

• Physical exam within 1 year of start.
• TDAP vaccine within 10 years of start (vaccine after age 19) or declination accepted.
• Negative TB test (PPD, QGold, or TSPOT) within 1 year; for positive responders, negative chest X-ray within 5 years and annual TB questionnaire required.
• Annual TB questionnaire.
• Proof of immunity or vaccination (or declination) for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (positive titer required; vaccine series alone not sufficient)
  • Varicella
• Negative 12-panel drug screen within 90 days of start.
• Seasonal flu vaccination or agency declination/exemption required during flu season.
• COVID vaccine preferred but not required; declination or exemption required if never vaccinated.
• Respiratory safety clearance required within 1 year of start and annually.
