Job Opportunity: Occupational Therapist - Home Health

Location: Moraine, Ohio
Employment Type: Travel/Contract
Contract Length: 8 weeks

Job Details

• Full-time position, Monday through Friday
• No weekends required, but flexible scheduling allows weekend visits if desired
• Weekly productivity expectation: 7 visits per day (35 visits per week)
• Geographic coverage includes Dayton/North Dayton, Kettering, Moraine, Middletown
• Patient population: Adults and geriatrics
• Electronic Medical Record (EMR) system used: Home Care Home Base (training provided if needed)

Required Qualifications

• Valid Occupational Therapist license in Ohio
• Minimum 1 year of Occupational Therapist experience within the last year

Compliance & Documentation Requirements

• I-9 and E-Verify documentation required
• Government-issued photo ID required
• 7-year criminal background check within 12 months, including SSN trace, national and county checks, and sex offender registry
• Education verification (primary source) required
• Physical exam within 2 years, indicating fitness to work and free of communicable diseases
• Negative TB test or chest X-ray documentation (annual updates if past positive)
• Required vaccinations or immunity documentation: Measles, Mumps, Rubella, Varicella, Hepatitis B (vaccination series, immune titer, or declination)
• Drug screening: Negative 9 or 10 panel drug screen within 12 months, including marijuana panel
• Seasonal flu vaccination or approved medical/religious exemption required during flu season
• Respiratory safety clearance within 1 year, renewed annually
• Two professional references within 2 years (one must be a manager/supervisor/charge RN) or employment verification if references unavailable
• Current resume or application required
• Skills checklist updated within 1 year
• Competency exams and other certifications as applicable

• Fingerprinting is not required for this position
• Prior travel experience is not explicitly required
• Training on EMR system provided if candidate lacks prior experience
