Physical Therapist (PT) - Home Health (Days)

Location: Pittsfield, Massachusetts

Contract Length: 13 weeks

Job Description

Provide physical therapy services to home health clients, including direct patient care, collaboration with team members, and participation in developing comprehensive patient care plans. Use rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, and ADL equipment. Modalities may also be used.

Required Qualifications

• Licensed Physical Therapist in Massachusetts (MA PT License)
• Home Health Certification (certified home health experience including proficiency with OASIS is non-negotiable)
• Minimum 2 years of acute nursing care (hospital-based) and home care experience
• Valid Driver’s License and must have own vehicle
• Ability to work independently with minimal supervision
• Active American Heart Association BLS certification
• Supervisory signed reference from within the past two years of employment
• Physical Therapist license current within 1 year

Additional Information

• Acute care experience preferred
• Must reside more than 50 miles from the facility (travelers residing within 50 miles will not be considered)
• Requested time off during the assignment must be submitted and approved at the time of profile upload; no requests accepted after offer
• No more than 5 days of requested time off will be accepted
• Must have voicemail set up and be reachable by phone during the application process

• This is a home health position requiring travel to patient homes
• Must be able to provide documentation of work history and skills checklist upon application
• Prior travel experience is not explicitly required
