Home Health/Hospice Registered Nurse (RN)

Location: Gunnison, Colorado
Employment Type: Contract
Contract Length: 13 weeks


Job Description

  • Provide patient care and case management using the nursing process and individualized Plan of Care for home health, hospice, and palliative care patients.
  • Administer skilled nursing care to patients requiring intermittent professional nursing services.
  • Conduct initial evaluation visits and regularly re-evaluate patients' nursing needs.
  • Initiate and revise plans of care as necessary.
  • Perform specialized nursing procedures and preventive/rehabilitative nursing interventions.
  • Prepare clinical and progress notes.
  • Coordinate services and communicate changes in patient condition to physicians and other personnel.
  • Counsel patients and families to meet nursing and related needs.
  • Participate in in-service programs and supervise/teach other nursing personnel.

Required Qualifications

  • Valid Registered Nurse (RN) license in Colorado
  • Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
  • Minimum 3 years of nursing experience
  • At least 1 year of home health nursing experience within the last year
  • Hospice nursing experience
  • Basic Life Support (BLS) certification
  • Colorado Adult Protective Services (CAPS) check required
  • Reliable transportation required due to limited public transportation in the rural area

Additional Information

  • Prior travel experience is required for this position.
