Physical Therapist - Outpatient

Location: Perry, Oklahoma
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide Physical Therapy treatments for patients with disease or disability.
• Deliver preventive health measures.
• Provide technical direction to students, therapy assistants, and technicians.
• Work independently in a small hospital setting.

Required Qualifications

• Valid Physical Therapist license in Oklahoma.
• Minimum 2 years of Physical Therapist experience, including experience within the last year.
• Bachelor of Science Degree from a school of physical therapy.

Additional Requirements

• Completion of background checks including a 7-year criminal history.
• Verification of education and licensure through primary sources.
• Compliance with health and safety standards including immunizations and drug screening:

  • Tdap vaccine within 10 years (declination acceptable).
  • Negative TB test or chest X-ray within 12 months.
  • Required immunizations or positive titers for Measles, Mumps, Rubella, Hepatitis B, and Varicella.
  • 10-panel drug screen within 1 year.
  • Current COVID vaccination or approved declination form.
  • Seasonal flu vaccination or approved declination form during flu season.
• Completion of facility-specific orientation and competency exams.
• Submission of a recent physical exam (within 1 year).
• Two professional references within 3 years.
• Government-issued photo ID and valid driver’s license.
• Passport-style photo for badge.

Notes

• Orientation includes HealthStream modules that must be completed prior to start.
• Fit testing and color-blind exam will be completed onsite during orientation.
• Candidate must review and sign facility-specific policies and standards prior to start.

This position requires a licensed Physical Therapist with at least 2 years of experience, able to work independently in a small hospital environment, and meet all licensure, certification, and health compliance requirements.
