Description
Job Opportunity: Hospital Pharmacist
Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 20 weeks (approximately 5 months)
Schedule:
- Rotating day/evening shifts, 40 hours per week
- Start times at 6:30 AM or 8:00 AM, Monday through Friday
- Rotating weekends working 7:00 AM to 12:00 PM
Job Description
- Prepare and dispense medication orders per physician requests.
- Maintain medication stock in patient care areas.
- Ensure safe, appropriate, and cost-effective medication therapies according to policies and protocols.
- Interpret medication orders and maintain computerized patient medication profiles.
- Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition products.
- Comply with all federal, state, and local laws and regulations, including controlled substances handling and record-keeping.
- Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and dose.
- Perform pharmacokinetic drug dosing per hospital protocol.
- Review and interpret culture and sensitivity data for antibiotic appropriateness and recommend changes as needed.
- Detect and report adverse drug reactions.
- Promote formulary adherence and rational drug therapy selection.
- Provide clinical consultation and drug information to practitioners, patients, and families.
- Document all clinical activities and interventions accurately.
- Participate in quality improvement and medication review activities.
- Collect data, conduct quality monitors and inspections, and maintain logs and records.
- Assist in orientation, education, and training programs for pharmacy, nursing, and medical staff.
- Serve as a member of the Code Response Team.
- Support facility EMR conversion from Meditech to Epic (Epic experience required).
Required Qualifications
- Valid Hospital Pharmacist license in Colorado
- Minimum of 3 years of hospital pharmacist experience, including experience within the last year
- BLS certification issued by the American Heart Association
- ACLS certification issued by the American Heart Association
- Experience with Epic electronic medical records system
Additional Information
- Traveler’s permanent address must be at least 75 miles away from the facility location.
- This is a travel assignment during an EMR system conversion.
This position is ideal for pharmacists with strong clinical skills, experience in hospital pharmacy operations, and proficiency in Epic EMR systems.
