Location: Grand Junction, Colorado
Employment Type: Contract

Job Description
The Registered Nurse (RN) in this role provides comprehensive, coordinated, and proactive whole-person care to elderly participants in a home care setting. The RN promotes and maintains participants' health through education, counseling, and coordination of preventive and rehabilitative services with an interdisciplinary team. Responsibilities include assessing health needs, overseeing and implementing nursing care plans, providing direct nursing care, and ongoing re-evaluation of individual and family needs. The RN also coordinates scheduling, communication, and documentation to ensure efficient home care services.
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum of 1 year of Home Health experience within the last year
- Experience in an Acute Care setting
