Job Opportunity: Physical Therapist - Home Health

Location: Demopolis, Alabama
Employment Type: Travel/Contract

Job Description

• Provide physical therapy services in a home health setting with home visits.
• Participate in periodic office meetings.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM.
• Weekend and on-call requirements vary; discuss during interview.
• Must be willing to travel up to 75 miles from the home clinic.
• Mileage reimbursed at IRS standard rate (patient home to patient home).
• Local candidates accepted.

Required Qualifications

• Valid Physical Therapist license in Alabama.
• Minimum of 1 year of recent experience as a Physical Therapist.
• Experience working with adult patients.
• Experience in a Home Health setting.

Additional Requirements & Compliance

• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months.
• Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
• Drug screening (10 panel) required.
• Seasonal flu vaccination or documented declination.
• COVID-19 vaccination series or declination required.
• Background checks including 7-year history, sex offender search, and state-specific screenings.
• Fingerprinting may be required depending on state placement.
• Education verification and work history verification (at least 2 most recent employers or last 7 years).
• Various state-specific and facility-required compliance documents and attestations must be completed prior to start.
• Must review and sign multiple compliance, confidentiality, privacy, and policy documents before starting assignment.

Travel Experience

• No explicit prior travel experience required, but must be willing to travel within the assigned territory.

Notes

• Guaranteed hours are not provided; if patient census is low, contractor may be floated to a nearby clinic.
• Weekend and on-call requirements vary by location and should be clarified during the interview.
• Candidates must be prepared to complete all required documentation and compliance training annually if working consecutively for more than one year.
