Job Description

• Position as a Registered Nurse case manager in a home care setting.

Required Qualifications

• Valid Registered Nurse (RN) license in Pennsylvania.
• Minimum of 1 year of Home Health experience within the last year.
• Current Basic Life Support (BLS) certification.
• Associate of Science in Nursing or Associate Degree in Nursing.
• Must meet all standard healthcare compliance requirements including: background check, drug screening, physical exam, immunizations (Hep B, MMR, TdaP, Varicella), TB screening (annual), and license verification.
• Must have a valid driver’s license and car insurance.
• Competency exam and skills checklist completion required.
• Compliance with COVID-19 vaccination policy.
• Completion of core mandatory annual training.
• Verification of education, employment, and references.
• Various screenings including FACIS 3M, NURSYS, SAM sanction, OFAC sanction, and National Sex Offender and Predator Register search.
• Fit mask test and color vision screening.
