Registered Nurse (Skilled Nursing Facility)
Location: Alton, Illinois
Employment Type: Travel/Contract
Contract Length: 16 weeks
Job Overview
- Work in skilled nursing and rehabilitation units within a long-term care (LTC) facility, functioning similarly to a Med-Surg environment.
- Provide direct patient care with ratios up to 20 residents per shift; average daily census is 72.
- Shift: Day shift, 3 shifts per week, 12 hours each (6:30 AM to 7:00 PM).
- Weekend rotation: Every other weekend required upon facility request.
- No on-call duties.
- First-time travelers are accepted.
- Locals are also accepted.
- Attire: Any shade of blue scrubs.
- Charting system: PowerChart.
- Free parking available; no mileage reimbursement.
Required Qualifications
- Valid RN license in Illinois
- Minimum 1 year of Long Term Care experience within the last year
- Skilled Nursing experience
- BLS certification
- ACLS certification
- IV Certification
- Associate of Science in Nursing / Associate Degree in Nursing
Compliance & Documentation Requirements
- Background checks required within 1 year of start, including SS trace, county searches (7 years), nationwide, sex offender, EPLS/GSA/SAM, and HHS/OIG checks.
- Professional license primary source verification required.
- Unit-specific competency exam and medication/pharmacology competency exam required.
- Physical exam within 1 year of start, updated annually (can be completed by NP or PCP).
- Immunizations/vaccinations required or documented declinations for:
- Tetanus/Diphtheria/Tdap (within past 10 years)
- Measles, Mumps, Rubella (positive titer or vaccination series)
- Hepatitis B (declination accepted)
- Varicella (vaccination, positive titer, or history of chickenpox)
- Seasonal flu vaccine or approved medical/religious exemption
- COVID-19 vaccine (fully vaccinated required) or approved medical/religious exemption with bi-weekly testing for Illinois candidates
- Negative TB test or chest X-ray per facility protocol.
- 9-panel drug screen within last 12 months (instant tests accepted; cannot start with pending drug screen).
- Two professional references within 2 years of start.
- Employment application and unit-specific skills checklist within 1 year of start.
- Photo for badge required prior to start.
Additional Notes
- Must provide ample notice (more than 24 hours) for any call-outs.
- Rehab experience with dementia and surgical patient care is a plus.
- No floating between units required.
- RTO (Request Time Off) restrictions apply: RTO only approved at time of submission; not approved after offer. Not all holidays can be taken off.
