Speech Language Pathologist (SLP) - Travel/Contract

Location: Bardstown, Kentucky
Setting: Home Health, Post Acute
Shift: Day shift, 8 hours per day (7:00 AM - 3:00 PM), 5 days per week
Hours: 40 hours per week
Contract Length: 13 weeks

Job Description

• Provide speech language pathology services in a home health post-acute care setting.
• Work within an AlliedCare division.
• Orientation: 12 hours.

Required Qualifications

• Minimum 1 year of experience as a Speech Language Pathologist.
• Valid license to practice as a Speech Language Pathologist in the state of Kentucky.
• Certified Clinical Competence in Speech-Language Pathology (CCC-SLP) via ASHA.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
• Must have a government-issued photo ID (Driver’s License or State ID).

Certifications & Compliance

• Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications are preferred.
• Background checks including 7-year work/school/location history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG must be completed and updated annually.
• Competency exams and medication/pharmacology tests as applicable.
• Physical exam within 1 year of start date.
• Negative TB test or chest X-ray within 1 year of start.
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, and Varicella as per policy.
• 9-panel drug screen within 1 year of start.
• Seasonal flu vaccination or declination required during flu season (October 1 - March 31).

Documentation & Other Requirements

• Emergency contact form required prior to start.
• Two professional references required for travel, allied, or per diem acute care roles (within 3 years).
• Annual skills checklist required.
