Description

Physical Therapist (PT) - Contract Position

Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks

Job Overview

Provide physical therapy services for elderly participants in a comprehensive care program aimed at maintaining and improving seniors' health to support safe living at home.
Conduct initial, annual, and condition-change assessments for participants.
Collaborate with an interdisciplinary team, participants, and families to develop and update individualized plans of care (POC).
Deliver clinical education and training to staff and volunteers to enhance participant safety and functional abilities during activities of daily living (ADLs).

Work Schedule

Monday to Friday, 8:00 AM to 5:00 PM

Required Qualifications

Valid Physical Therapist license in Colorado
Minimum of 1 year of Physical Therapist experience within the last year
Current Basic Life Support (BLS) certification

Skills and Competencies

Proficient in Microsoft Word, Excel, Access, and Outlook.
Excellent interpersonal and communication skills, able to communicate effectively and respectfully in various situations, including emotionally charged interactions.
Ability to work effectively with others and build strong professional relationships.
Comfortable working in diverse environments.

Additional Information

No prior travel experience required.
