Registered Nurse (RN) – Home Health (Days)

Location: Kāne‘ohe, Hawaii
Employment Type: Temporary
Contract Length: 13 weeks

Job Description

• Provide excellent patient care by collaborating with patients, families, nurses, physicians, and multidisciplinary teams to optimize outcomes.
• Act as a consultant to the healthcare team on specific patient issues.
• Lead coordination of patient care with other disciplines, ensuring appropriateness and timeliness of care.
• Monitor interdisciplinary care plans to align with patient clinical course, continuing care needs, and covered services.
• Discuss resource utilization, treatment plans, length of stay, and discharge plans with physicians.
• Focus on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients.
• Collaborate with acute care case managers to ensure ongoing care appropriateness.
• Coordinate patient transfers to tertiary centers, ensuring continuity of treatment and services.
• Provide oversight and collect data for regulatory and accreditation compliance.
• Manage frequent emergency department visitors through focused review of admissions.

Required Qualifications

• Active and clear Hawaii RN license.
• Minimum 2 years of Medicare-certified home health agency experience.
• Prior travel experience required.
• Active and clear driver’s license and car insurance.
• Proficiency with Homecare Home Base and OASIS documentation systems.
• COVID-19 and flu vaccinations required (no declinations accepted).

Preferred Qualifications

• Bachelor’s degree.
• Case management experience.

Work Schedule

• Day shift, 8 hours per day.

Additional Information

• Mileage reimbursement provided (rate below IRS standard).
• Assignment territory will be provided close to start date due to changing needs.
