Job Opportunity: Home Health/Hospice Registered Nurse (RN)

Location: Gunnison, Colorado
Employment Type: Contract
Contract Length: 13 weeks

Job Description

Provide patient care and case management for home health, hospice, and palliative care patients.
Conduct initial evaluation visits and regularly re-evaluate patients' nursing needs.
Initiate and revise plans of care as necessary.
Deliver specialized nursing services and preventive/rehabilitative nursing procedures.
Prepare clinical and progress notes.
Coordinate services and communicate changes in patient condition to physicians and other personnel.
Counsel patients and families regarding nursing and related needs.
Participate in in-service training programs.

Work Schedule

Day shift.
On-call requirements: 1-2 nights per week and 1-2 weekends per month.
Approximately 36 hours per week.

Required Qualifications

Valid Registered Nurse (RN) license in Colorado.
Basic Life Support (BLS) certification.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Minimum 1 year of hospice experience within the last year.
Home health nursing experience.

Additional Information

This position involves working in a rural setting.
Prior travel experience is required.
