Description
Physical Therapist - Home Health (Travel/Contract)
Location: Portland, Oregon
Contract Length: 13 weeks
Job Overview
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points expected weekly).
- Types of cases: Start of Care and Follow-Ups.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have alternate schedules such as 4x10 shifts (Friday-Monday).
- No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged.
- Must be willing to travel up to 75 miles from the home clinic, with an estimated 200 miles traveled weekly. Mileage reimbursed at IRS standard rate (patient home to patient home only).
- Local candidates accepted at the same rate.
Required Qualifications
- Valid Physical Therapist license in Oregon.
- Minimum of 1 year of recent physical therapy experience within the last year.
- Experience working with adult patients in a home health setting.
- Current BLS certification.
Additional Requirements & Expectations
- Weekend and on-call requirements vary by location; discuss during interview.
- Use of EMR system: HCHB.
- Must complete various compliance, confidentiality, and policy reviews prior to start, including but not limited to:
- HIPAA and Privacy Policy
- Compliance Training and Code of Conduct
- Home Health Scenarios and Core Policy Acknowledgements
- COVID-19 Policy and Attestation
- Background checks and verifications required, including:
- 7-year standard background check with multiple state and federal components
- Drug screening (10-panel)
- Education verification
- State-specific registry and fingerprinting requirements as applicable
- Health documentation required prior to start and updated annually:
- Physical exam within 12 months confirming good health and no communicable disease
- Negative TB test or chest X-ray within 12 months
- Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
- Flu vaccination or documented declination for current season
- COVID-19 vaccination series or declination
- Must provide two professional references and work history verification covering at least the last 7 years or two most recent employers.
Notes
- The need for this contractor is driven by referral demand and onboarding/training of new hires.
- Candidates must sign new supplier packets at the start of each assignment and annually if working more than one consecutive year.
- Fingerprinting and certain state-specific clearances may be required prior to start depending on the assignment location.
- Candidates should be prepared to complete all required documentation and compliance training promptly to avoid delays.
This position is ideal for experienced physical therapists with recent home health experience who are comfortable with travel and working independently in patient homes.
