Pharmacist Inpatient - Travel/Contract Position
Location: Wichita Falls, Texas
Contract Length: 13 weeks
Job Details
- Employment Type: Travel/Contract
- Shift: Day shift, 4 days per week, 10-hour shifts (12:00 PM to 10:30 PM)
- Schedule: Weekdays with variable weekends required
- Start Date Preferred: July 27
Required Qualifications
- Valid Clinical Pharmacist license in Texas
- Minimum of 1 year of Clinical Pharmacist experience within the last year
- Experience in an inpatient setting
Compliance and Documentation Requirements
- Background Check: Must be completed within 1 year of start, including SS Trace, County searches (7 years), Nationwide, and Sex Offender checks; updated annually
- Federal Exclusions Checks: EPLS/GSA/SAM and HHS/OIG required within 30 days of start
- Identification: Government-issued photo ID required
- Employment Eligibility: I9 or Attestation and E-Verify or Attestation required
- Physical Exam: Required within 1 year of start; must be completed within 72 hours of contract signing
- Drug Testing: Negative 9 Panel Drug Screen within 1 year of start; plan for completion within 72 hours of contract signing required
- Immunizations and Health Requirements (must be current or have approved exemptions):
- TDAP vaccine within 10 years
- Negative TB test (2-step PPD, Q Gold, or TSpot) within 1 year or chest x-ray within 5 years if positive PPD
- Rubeola (Measles): 2 vaccines or positive titer (exempt if born before 1957)
- Mumps: 2 vaccines or positive titer (exempt if born before 1957)
- Rubella: 2 vaccines or positive titer (exempt if born before 1957)
- Hepatitis B: Positive titer, series of 3, or declination accepted
- Varicella: 2 vaccines, positive titer, or history of chickenpox
- Seasonal Flu Vaccine required during flu season (Oct 1 - May 31) or approved medical/religious exemption
- COVID-19 vaccination documentation required
- Orientation and Facility-Specific Documents: Must complete all required facility-specific forms and orientation prior to start
- References: 2 references required within 3 years of start
- Employment Application: Resume or application copy required
- Skills Checklist: Must be updated annually
