Description
Occupational Therapist (OT) - Long Term Care (LTC)
Location: Ada, Oklahoma
Employment Type: Temporary
Contract Length: 13 weeks
Shift: Days

Job Description
- Screen and evaluate patients with cognitive and/or ADL (Activities of Daily Living) physical functioning disorders.
- Develop and implement appropriate plans of care following regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive/assistive devices.
- Assess patient progress and adjust treatments accordingly; evaluate treatment outcomes and modify services as needed.
- Provide consultation and counseling to patients, families, caregivers, and other service providers.
- Communicate effectively with supervisors and healthcare team members regarding patient progress, barriers, and treatment plans.
- Document all required information in electronic records and maintain patient records per company policy.
- Accurately record treatment minutes in compliance with regulatory and clinical requirements.
- Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and required trainings or in-services.
- Obtain necessary durable medical equipment (DME) or supplies as needed.
- Supervise and support occupational therapist assistants, rehabilitation aides, and/or students.
- Adhere to NBCOT and State practice acts, code of conduct, and ethics during professional practice.
Required Qualifications
- Minimum 1+ years of Occupational Therapist experience.
- Experience in Long Term Care (LTC) or Rehabilitation settings.
- Active Occupational Therapist license in Oklahoma.
- New graduates considered if they have completed Level II fieldwork experience (not just Level I observation).
- Resume, skills checklist (within 1 year), and 0-1 professional reference required at time of submission.
- Must have an active OT license at time of submission.
Additional Information
- Emphasis on enabling individuals to reach their highest functional level while preserving dignity and well-being.
- Work schedule is 8 hours per day during the day shift.
- Participation in required meetings and trainings is expected.
- Supervision responsibilities include occupational therapist assistants and rehabilitation aides.
