Description
Job Opportunity: Occupational Therapist - Acute Care Hospital
Location: Kingman, Arizona
Employment Type: Travel/Contract
Contract Length: 16 weeks
Shift: Day shift, 4 days per week (Friday to Monday), 10 hours per day (7:00 AM – 5:30 PM)
Work Setting: Acute care hospital
Patient Diagnoses: Mild stroke, weakness, gait issues, fractures, orthopedic surgeries (all traumas are transferred out)
Required Qualifications
- Valid Occupational Therapist license in Arizona
- Minimum 1 year of Occupational Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification
- National Board for Certification in Occupational Therapy (NBCOT) certification
Documentation and Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Background checks including:
- 7-year search within 30 days of start (updated annually) covering SS trace, county searches, nationwide, sex offender registry, education verification
- EPLS/GSA/SAM and HHS/OIG checks (required annually)
- Education verification: Transcript or diploma required; if unavailable, National Student Clearinghouse (NSCH) verification acceptable
- Primary source online verification of state license
- Competency exams and medication/pharmacology tests as applicable
- Physical exam within 1 year of start
- Negative TB test or chest X-ray within 1 year of start; annual TB questionnaire required
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella (Chickenpox)
- Drug screening: Negative 9-panel drug screen within 30 days of start (instant drug screen acceptable)
- Seasonal flu vaccination or declination during flu season (October 1 – March 31)
- Two professional references within 3 years of start
- Annual skills checklist
Additional Facility-Specific Requirements
- Clear, passport-style photo with solid background for badge
- Completion of facility-specific forms including:
- Security and confidentiality agreement
- Behavioral expectations
- Right to know
- HIPAA and information security training
- Agency attestation and candidate forms
Summary
This is a travel Occupational Therapist position in an acute care hospital setting, focusing on patients with mild stroke, orthopedic conditions, and mobility impairments. The role requires a minimum of one year of experience, appropriate state licensure, and BLS/NBCOT certification. The schedule is four 10-hour day shifts per week, Friday through Monday. Comprehensive background checks, health screenings, and facility-specific documentation are mandatory prior to start. Prior travel experience is not required.
