Radiology Tech - Waterloo, Iowa
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 10 weeks
- Shift: Monday to Thursday, 8:00 PM to 6:00 AM
Required Qualifications
- Valid Radiology Technician license in Iowa
- Minimum of 1 year of Radiology Technician experience within the last year
- Fit Test
- Trinity Specialty Specific Competency Testing
- Trinity Joint Commission Testing
- Trinity Child and Adult Dependent Abuse Training
Required Onboarding Documentation and Compliance
- License/certification verification dated within 30 days of start.
- Fit test.
- 10-panel drug screen completed within 30 days of start (no THC).
- Signed Code of Conduct form.
- Completed I9 form.
- Confidentiality and Information System Security Agreement.
- Integrity and Audit Services form signed shortly before start.
- Non-Employee Upload Form fully completed.
- Color vision test.
- COVID-19 vaccine proof (including booster by Sept 2025), declination or exemption accepted except in CA or IL.
- MMR vaccination proof (2 doses or IgG titer).
- Hepatitis B vaccination series started prior to start date.
- 2-step PPD or IGRA test for TB; if positive, chest x-ray per regional requirements.
- Varicella vaccination proof or IgG titer.
- Physical/Health statement confirming fitness to work within the last year.
- Criminal background check (federal, nationwide, county) covering last 7 years, dated within 30 days of start.
- Education verification of highest academic level.
- EPLS (Excluded Parties List System) check dated within 30 days.
- OIG (Office of Inspector General) exclusion check dated within 30 days.
- Joint Commission CORE exam within the last year.
- NSO (National Sex Offender) check dated within 30 days.
- System Access Form completed and uploaded one week before start.
- Skills checklist provided by agency for clinical contractors.
- Current, non-expired Tdap vaccine or declination if no pediatric interaction.
- Child and Adult Dependent Abuse Training (Iowa only).
- Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries) within 30 days prior to start.
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days.
Additional Notes
- Resume, skills checklist (within one year), and two professional references required for submission.
- All required documents must be uploaded to the credentialing system prior to onboarding.
- No prior travel experience explicitly required for this assignment.
