Physical Therapist Position

Location: Bogalusa, Louisiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks
Shift: 8:00 AM – 4:30 PM

Job Description

• Provide physical therapy services as a licensed Physical Therapist (PT).

Required Qualifications

• Valid Physical Therapist license in Louisiana.
• Minimum of 2 years of Physical Therapist experience, including experience within the last year.
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.

Pre-Employment Requirements

• Submission of a current resume/profile.
• Completion of a criminal background check within 30 days prior to start, including:

  • SSN address trace report covering all counties lived/worked in the last 7 years.
  • County criminal searches for all relevant counties (state checks not accepted).
  • National criminal check including NSO, OFAC, OIG, and SAM databases.
  • Education verification by a third party.
  • Employment verification for the last 5 years.
  • Driving record check (may be included or separate).
• Negative 10-panel drug screen within 30 days prior to start (includes marijuana/THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone). Positive marijuana/THC results reviewed case-by-case.
• Submission of a badge photo (forward-facing, well-lit, neutral background, JPEG format).
• Completion of annual education packet and competency exams with passing scores of 80% or higher within one year prior to start.

Additional Notes

• COVID-19 vaccination or exemption is optional.
• All declinations for immunizations or vaccines require appropriate signatures and approvals.
• Documentation must be current and compliant with annual renewal requirements.
